By submitting this form, the Applicant certifies that (a) the Applicant has reviewed the NHTA Product of the Year Awards terms and conditions contained in this form and, if applicable, any additional terms and conditions posted on the NHTA website, and (b) the Applicant has complied and agreed with them. The undersigned represents that he or she has been granted authorization to submit this entry form on behalf of the Applicant. The information provided in this 2024 NHTA Product of the Year Awards entry form is true and accurate. The Applicant agrees that NHTA is not liable for any loss of business or other loss or damage that the Applicant may suffer or incur in connection with or due to the Applicant’s participation in or the results of the competition. The Applicant also waives any and all claims against NHTA, and its judges, directors, officers, members, employees, agents, and representatives, that are in any way related to or arise out of such participation or results.

It is understood and agreed that NO CONFIDENTIAL OR PROPRIETARY INFORMATION SHALL BE PROVIDED BY THE APPLICANT. Neither NHTA, its judges, nor anyone else shall be prohibited or otherwise restricted in any manner from, nor liable for, disclosing any of the information provided in this application, in any presentation, or otherwise in connection with NHTA’s Product of the Year competition. In addition, there shall be no prohibition or other restriction on, nor liability for, the use of any such information, except to the extent that usage is protected by applicable patent, copyright, or trademark laws, none of which are waived.
